Changing Homelessness serves as the lead agency for the U.S. Department of Housing and Urban Development’s Continuum of Care (CoC) Grant in Northeast Florida (Duval, Clay, and Nassau Counties). They coordinate funding and delivery of housing services for individuals and families experiencing homelessness, including veterans, through a network of 50 member agencies. Services include housing search and identification assistance, budgeting support, and connections to benefits via the SSVF program. They manage the Homeless Management Information System and provide training to partner agencies.
